Starting from September 15 2011, visa applications for Schengen and National visas submitted in all Polish Consulates must be previously electronically registered on the website:

Validity remaining on Passport At least 3 months longer than the visa applied for.
Other Passport requirements At least two blank pages for the visa sticker.  Passport should also have been issued within the last 10 years.
Student Visa validity At least 3 months longer than the visa applied for.

However, you must apply in person unless the Embassy is more than 200 miles from where you live.  Applications can also be submitted by family members.

Application form P Download from website.
Photographs P Two recent passport-size colour photographs.
Bank Statements P Recent, showing name and address, and sufficient funds to support yourself for the duration of your stay.
Department Letter    
Evidence of return tickets    
Evidence of Hotel booking P Confirmed booking of accommodation for the duration of your stay in Poland.
Travel/medical Insurance P Minimum cover of €30,000.
 
Visa fee: €60 (£52, subject to exchange rates).
Payment Method: Cash or Postal Order only. Credit/Debit cards or Personal Cheques cannot be accepted.
