Starting from September 15 2011, visa applications for Schengen and National visas submitted in all Polish Consulates must be previously electronically registered on the website: |
| Validity remaining on Passport | At least 3 months longer than the visa applied for. |
| Other Passport requirements | At least two blank pages for the visa sticker. Passport should also have been issued within the last 10 years. |

However, you must apply in person unless the Embassy is more than 200 miles from where you live. Applications can also be submitted by family members. |
