Once downloaded, complete this form on computer, print up, sign it, acquire your supervisor’s signature (for student research projects) and return three copies to the Ethics Secretary, School of Education, Room 0G 018, 20 College Green.
All researchers should consult the following materials when planning and conducting their research:

A summary of the most frequent causes for substantive amendments and resubmission of applications may help you to complete the form comprehensively and appropriately (feedback to supervisors and students);

Information about the policies and procedures under which the School of Education’s Research Ethics Committee operate (School of Education Policy and Principles on Ethics in Educational Research).

Students should note that the cut-off dates for submission of completed forms to the Committee for 2011/12 are: 14th October; 2nd December; 17th February; and 11th May.
