If you are declining the offer, you need only complete Part A of the Offer form and return it to us. Please note that we do not defer places. If you are unable to accept the offer this year you will have to re-apply for entry in September 2014.

In order to enable schools to provide you with the most appropriate experience, you should complete the student profile form which will be sent directly to the schools we approach requesting a placement. Given that this is the first impression a school has of you, please complete this fully, checking the accuracy of your spelling and grammar. Present yourself in a favourable light – treat this as if it were an application form as schools may agree to a placement or refuse you on the basis of the information you provide in your profile. The student profile form should be returned by email.
Before completing the Access NI Enhanced Disclosure Form please read the ‘Notes for New Entrants’ and the 'Guidance for Completing Enhanced Disclosure Application’. You should complete sections B, D, E, F, G of the Disclosure Certificate Application and return in hard copy, along with:
As we already hold your GCSE or equivalent examination certificate from Group 2, you need only send one document from Group 1 and one from Group 2. Please include both photographic and paper parts of your driving licence if producing this form of I.D.
Each applicant must complete and return the Declaration of Health. Dates of most recent vaccinations must be given to avoid delay in processing your medical information. You may need to contact your GP or University Health Department for this information. The completed questionnaire should be returned directly to the School of Education in hard copy and we will forward it to the Senior Medical Officer, University Health Service. Please read carefully the notes which accompany the declaration of health questionnaire.
