Below is a backup copy of the Queen's forms that you will have to complete either before, during or after your Erasmus placement.  Some of the forms have to be countersigned by your Erasmus Programme Director or Placement Co-ordinator at Queen's, others by your host university (study placements) or host employer (work placements).

You should normally obtain the Erasmus Grant Application Form from your Erasmus Programme Director or Placement Co-ordinator in your School.  Once the Erasmus Grant Application Form is submitted to the Global Opportunities Team, we will send you the other forms that are required for your study or work placement.  The forms below are simply backup copies that you can use if you misplace those sent to you by the Global Opportunities Team.
