When any new website is ready to be published live, the following form needs to be submitted by the website owner/editor.  This form focuses on accessibility requirements. The checklist in the form will help you ensure that the content created in your website adheres to accessibility regulations and content usage requirements.

As the website owner/editor, you will be required to answer and submit this Microsoft Form to confirm that your website content is ready to go live.
