Exercise in Advanced Prostate Cancer

‌

Project Title

Programme of research focussing on exercise in advanced prostate cancer.

Two projects:

  1. Movember GAP4: INTERVAL – MCRPC: INtense Exercise for survival with Metastatic Castrate-Resistant Prostate Cancer: a multicentre, randomised, controlled, phase III study.
  2. EXACT-MCRPC: Exercise for advanced prostate cancer: a multicomponent feasibility trial in men with metastatic castration resistant prostate cancer.

Research Focus:

Chronic Illness and Palliative care

Description of Project:

Aim; Methods; Expected Outcomes

(up to 300 words)

Through a global collaboration, Movember are executing a large scale multi-centre trial designed to test the effects of exercise on prostate cancer progression and treatment side effects.  The project is underpinned by a global translational research initiative (GAP4) which aims to better understand the biological mechanisms of action of exercise on prostate cancer. Patient data and biological samples will be pooled and studied across the world by leading scientists and clinicians. To be able to definitely address the question of exercise on survival, large numbers of participants are necessary, this is why it is essential to conduct the study on a global scale. The Northern Ireland Cancer Centre (NICC) has been provided with the opportunity to act as a recruitment site to contribute to this global study, and has been tasked with recruiting 45 men with MCRPC to the study over the next three years (with a two year intervention period).

To provide evidence on the feasibility of exercise interventions in patients for whom high intensity exercise is not suitable we are running a parallel Northern Ireland specific study based upon an exercise intervention (EXACT) developed and recently tested in colorectal cancer survivors  by colleagues from Ulster University.

EXACT-MCRPC will offer the benefits of participating in a multicomponent physical activity programme to those men with mCRPC who are ineligible for the INTERVAL programme i.e. high intensity exercise. We wish to ensure all men have the opportunity to capitalise on the benefits of increased physical activity by offering a lower intensity lifestyle physical activity intervention. This is the first of its kind in this advanced and unwell population. This feasibility study will provide preliminary evidence on the acceptability, feasibility and efficacy of moderate intensity physical activity among men with very advanced cancer, and setting the benchmark for all other cancer patients.
